To provide us with the necessary information to accurately prepare your return, please click on the "Assessment Form" shown below. This will bring you to a "word" formatted document. Once completed, you can save it for your own record of the information provided and then, either e-mail it to us as an attachment or fax it to our secure fax line.
If your return is standard, you can complete Question 1 to 9 only of the "Assessment Form" and fax copies of this page and your documents (T4's, RRSP receipts, etc.) to us on our secure fax line. If we need further information, we will contact you, so please provide a telephone number and the best time to call.
Please note that unless advised otherwise, we will assume that you have authorized Canada Revenue Agency to provide your name, address and date of birth to Elections Canada for the National Register of Elections. This information will be used only for the purposes permitted under the Canada Elections Act.
